Optical factors in increased visual acuity after excimer laser surgery performed on mixed astigmatic patients using bitoric ablation: vector analysis

Purpose:
The main purpose of this study is to assess the possible optical factors resulting in increased best-spectacles-corrected visual acuity after excimer laser surgery performed on mixed astigmatic patients using bitoric ablation..
Setting:
This study was executed in the years between 2004 and 2010 using the data of 57 eyes of 35 patients that are strict to their follow-up visits.
Methods:
The patients went under either surface ablation or LASİK procedure and examined regarding their visual acuity, refractive errors and wavefront aberrations. All surgeries were planned and performed by the same surgeon using the Nidek Advanced Vision Ekzimer Laser System and Nidek MK-2000 microkeratome in Cornea and Refractive Surgery Division of Gulhane Military Medical Academy Ophthalmology Clinic.
Results:
The mean age was 29.32+7.12. Eyes with BCVA of 0.3 logMAR(20/40) or better was %100, of 0.00 logMAR(20/20), %56.1 and of - 0.1 logMAR(20/16), %21.1. There was a statistically significant increase in the mean BCVA from 0.19logMAR + 0.12 to 0.03logMAR + 0.12 and UCVA from 0.66 logMAR+0.31 to 0.1 logMAR+0.1. 47 eyes(%82.4) gained 1 or more lines of BCVA. None of the patients lost visual acuity. Eyes with astigmatism of +0.50 D was or better was % 22.8, of +1.00 D, %54.3 and of +2.00D, %94.7. There was a significant increase in total high order aberrations after surgery but, total coma and total trefoil didn’t change. Vector analysis showed significant decrease in blurring strength(p<0.01)
Conclusions:
Bitoric excimer laser ablation is an effective and safe method in the treatment of mixed astigmatism. Sparing the corneal asphericity and improving the optic quality after correction of mixed astigmatism, results of visually impaired patients seems to be better.
