Purpose:
To evaluate diagnostic efficacy of Van Hericks grading and angle parameters on Anterior Segment Optical Coherence Tomography (AS OCT)
Setting:
Te study was conducted in Laxmi Eye Institute, Panvel, Maharashtra, India
Methods:
Secondary data analysis comparing efficacy of angle parameters on AS OCT and their correlation with Van Herick’s grading of peripheral anterior chamber depth in diagnosis of occludable angles. The gold standard for angle evaluation was gonioscopy.
Results:
69 eyes of 39 patients evaluated. Mean age was 54.5 yrs (SD 15.8). The sensitivity and specificity of Van Hericks alone with grade 2 as cut off was 76.2% & 68.4% respectively. With a van herick grade of 2 or less and angle on OCT of 23.5 degrees or less the diagnosis of occludable angle improved (specificity 83.3% [95% CI 69.8-92.5] & sensitivity 66.7% [95% CI 43-85.4%], NPV 85.1%). If either of the parameters were present then the sensitivity increased to 90.5% [95% CI 69.6 – 95.8] & specificity was 60.4% with a NPV of 93.5 [95% CI 78.6 -99.2]
Conclusions:
Van Hericks method and angle on AS OCT in combination provide a sensitive, rapid & non-invasive method of angle evaluation.
