Anterior segment anatomical changes with soft multifocal contact lenses assessed by optical coherence tomography

Purpose:
To assess the changes in the anterior segment while wearing soft multifocal contact lenses for presbyopia by means of optical coherence tomography (OCT).
Setting:
University of Valencia, Burjassot, Valencia, Spain.
Methods:
Five right eyes of five healthy adults (mean age 28.3 ± 2.9 years) were included. The device used for the measurements was the Visante® OCT system (Carl Zeiss AG, Oberkochen, Germany). The central corneal thickness (CCT), anterior chamber depth (ACD), central lens thickness (CLT) and anterior segment length (ASL) were assessed. The parameters were obtained with no lens, and with two soft multifocal contact lenses of different addition at stimulus vergences 0 D, -1.5 D and -3 D. The variation of these parameters was compared for the lenses with respect to the naked eye condition at the same stimulus vergence.
Results:
The CCT was not practically altered at any stimulus vergence while wearing the lenses with respect to not wearing them. The ACD showed a lower reduction at -1.5 D and -3 D while wearing the lenses and CLT showed a lower increase. The change in ASL was slightly smaller than the change in ACD and CLT parameters.
Conclusions:
Wearing soft multifocal contact lenses even in patients with preserved functional accommodation produce differences in the anatomical structures in the anterior segment that are easily observed by means of the OCT technology. This paper shows a methodology in order to evaluate how soft multifocal lenses contribute to the patient’s vision by analysing the anatomical changes of the anterior eye.
