Corneal optical aberrations in fistulizing glaucoma surgery

Abstract Details
Purpose:
To assess types and dynamics of development of progressing, regressing, stable corneal aberrations of lower and higher orders in the early and late periods after filtering surgery for glaucoma.
Setting:
Irkutsk Branch of S. Fyodorov Eye Microsurgery Federal State Institution, Irkutsk, Russia
Methods:
Exanimated 63 people (75 eyes) underwent fistulizing trabeculectomy. Corneal Analysis Zernike on Pentacam apparatus (Oculus Inc., Germany) and keratorefractometry (8800 Topcon, Japan) were made before surgery and in 5 day and 1, 3, 6 and 12 months. The dynamics of corneal optical aberrations depending on IOP level, terms of the postoperative period and the type of formed filtering bleb was assessed.
Results:
When IOP reducing in the early postoperative period a significant increase in spherical aberration (P = 0.053), vertical coma (p = 0.0017), horizontal coma (p = 0.0014), trefoil (p = 0.00026) was revealed. After 6 months at IOP of 10 - 12 mmHg and diffuse comfortable filtering bleb the higher order aberration decreased to baseline. In the formation of scar transformation and failing bleb the emergence of lower-order aberrations - irregular astigmatism (p = 0.001) was observed. Refractometry revealed irregular astigmatism ranged from 1.75 to 3.75 diopters, which caused visual acuity decrease and impossibility of optimal correction.
Conclusions:
In adequate formation of outflow pathways in early postoperative period the induced anomalies of refraction and aberrations are reversible. It is necessary to consider severe optical aberrations of lower order in scar transformations of filtering bleb tissue as indication for reconstructive surgery.
