A complication of deep non-penetrating sclerectomy: hematoma in scleral lake

Abstract Details

Purpose:

We show a rare complication after Deep Nonpenetrating Sclerectomy (DS) and its subsequent treatment.

Setting:

12 de Octubre Universitary Hospital, Complutense University, Madrid, Spain.

Methods:

We described four patients suffering from open-angle glaucoma and cataract, and one patient with secondary glaucoma after Fuchs’ heterochromic uveitis. All of them underwent phacoemulsification and DS with Esnoper® implantation. One patient presented hematoma in scleral lake 48 hours after surgery, three of them presented it one week after the intervention, whereas the remaining patient presented it after two weeks. The hematoma was found by gonioscopy in all cases. Moreover, all patients had elevated intraocular pressure (IOP).

Results:

Hematoma usually appears within two weeks after surgery. Goniopunctures were performed in five patients after one month of the complication. A second goniopuncture was needed for one of them. All cases required treatment with beta blockers eye drops since IOP exceeded pre-set values.

Conclusions:

Hematomas in scleral lake are a rare complication that may appear within two weeks after DS. They may cause a surgery malfunction and antihypertensive eye drops are required for tensional control. Goniopunture could be used for resolve the complication.
