Purpose:
Trabeculectomy is the mainstay of surgical therapy in order to control the intraocular pressure (IOP) in long term. The healing process is crucial in glaucoma surgery and Mitomycin C and Ologen implants help reduce the formation of scarring tissue. We present a single institution one year outcomes of trabeculectomies with MMC and Ologen in regards to visual acuity (VA) and intraocular pressure.

Methods:
Since 2007 a total of 300 patients (453 eyes) underwent trabeculectomy using Mitomycin C and Ologen implants. 128 were males and 172 females. The age distribution was from 29 to 92 years. In 104 patients trabeculectomy was combined with cataract surgery. An assessment including measurement of the corrected visual acuity, as well as Goldmann applanation tonometry was performed preoperatively and postoperatively as follows: 1. postoperative day, 2-4 weeks, 3-, 6- and 12-months. We monitored for choroidal folds, positive Seidel test und therapeutic management with ocular digital massage, anterior chamber reformation, suture manipulation, needling or re-trabeculectomy are reported.
Results:
We found no statistically significant difference of the VA between the group of patients who underwent trabeculectomy and the controls. The group of combined surgery showed a notable increase in visual acuity within 3 months. In all groups the IOP was lowered significantly. There was no difference in terms of lowering the IOP between the groups of patients who were treated either with combined surgery or only with trabeculectomy. Serous choroidal detachment was more prevalent in the first postoperative months. During the whole surveillance period 34 patients required chamber reformation, 19 bleb - resuturing, 3 suturolysis and 20 re-trabeculectomy.
Conclusions:
Our data demonstrate a satisfying long term control of the intraocular pressure (93,88% of the patients showed a pressure of < 15 mmHg) as well as a stabilization of the visual acuity. The complication rate was relatively low.
