Bilateral single piece syndrome: clinical and surgical challenges in diagnosis and management

Abstract Details

Purpose:

Authors present 2 cases of uveitis glaucoma hyphema (single piece) syndrome that were difficult to diagnose due to late presentation, associated systemic pathologies (secondary trombocytopenia in hepatitis C and non Hodgkin lymphoma in remission) or chronic blood thinner agents for cardiac pathology combined with degenerative myopic changes, where fundus evaluation could be misleading for glaucoma excavation assessment

'Sf. Spiridon' University Hospital, Ophthalmology Department

Methods:

Chart reviews of 2 patients with sulcus placed IOL presenting with UGH syndrome and reverse pupillary block with posterior iris bowing as diagnosed by gonioscopy, anterior segment optical coherence tomography and ultrasonic biomicroscopy.

Results:

Due to numerous inflammation episodes that were unresponsive to topical medication, recurrent microhyphema in one case and hyphema plus vitreous hemorrhage in the other case, the decision was to explant the single piece IOLs and replace them with a sulcus placed multipiece IOL. The main outcome was clinical resolution of UGH syndrome after IOL replacement

Conclusions:

Late presentation after aproximately 10 years and associated systemic pathologies complicated the clinical picture of a UGH syndrome (single piece syndrome) which was sucessfully managed after IOL replacement.
