Purpose:
To evaluate the clinical outcomes of Ahmed Glaucoma Valve implantation in refractory glaucoma
Setting:
A total of 72 eyes of 72 patients with refractory glaucoma were treated by Ahmed Glaucoma Valve FP7 implants. The average age was 67±18,58 years and the follow-up period was from 8 months till 24 months. Prior glaucoma surgery had failed in 21 (29.2%) eyes.
Methods:
Their baseline clinical data including the visual acuity (VA), intraocular pressure (IOP) and the number of anti-glaucoma medications were recorded. Postoperatively, VA, IOP and number of anti-glaucoma medications and any complications were recorded. Success was defined as an intraocular pressure (IOP) below 21 mmHg with or without medications and without serious complications or additional glaucoma surgery.
Results:
The mean preoperative IOP was 37,29±3,75 mm Hg and a mean number of topical medications was 2.2 ± 1.5. In the early postoperative period the average IOP was 10±0,23.. Two eyes developed a shallow anterior chamber with choroidal detachment at first week, two of which resolved spontaneously with medications and one required surgery. Hyphema occurred in 6 eyes, it resolved spontaneously during the first postoperative week in 5 eyes and during 2 weeks in one eye. At last follow-up the mean IOP and the number of medications were 18.5±2.5 mmHg and 1.3±1.23, respectively. 7 patients had no significant improvement of IOP at follow-up.
Conclusions:
Ahmed Glaucoma Valve FP7 reduces IOP and topical medications use in a significant proportion of cases The most frequent complication was development of hyphema which spontaneously resolved in most eyes.
