Changes in peripapillary choroidal thickness after non-penetrating deep sclerectomy

Abstract Details

Purpose:

To evaluate changes in peripapillary choroidal thickness (pCT) and other optic nerve head (ONH) structures such as the prelaminar tissue (PT) thickness, lamina cribrosa (LC) thickness and depth and optic disc cupping after deep sclerectomy (DS) and to analyze the relationship between these parameters and intraocular pressure (IOP) reduction, peripapillary retinal fiber nerve layer (pRFNL) thickness and axial length (AL) in order to understand its potential role in the pathophysiology of glaucoma.

Setting:

Hospital Ramón y Cajal, Department of Ophthalmology, Madrid.

Methods:

Prospective observational study involving glaucomatous patients undergoing DS. The ONH was evaluated using Spectralis OCT (Heidelberg Engineering GmbH, Heidelberg, Germany) and the EDI technique. PCT was automatically measured with SS-OCT (OCT DRI Atlantis, Topcon, Japan). All OCTs were performed before surgery and at 1 week postoperatively by a single masked operator. Sex, age, AL (IOLMaster, v.4.1, Zeiss), mean pRFNL thickness and IOP were registered. Statistical analyses were performed using SPSS software (version 19.0, Chicago, IL, USA). Given the sample size nonparametric tests were used. P<0.05 was considered significant.

Results:

18 eyes of 18 patients (66.2% women, mean age 61.9±14.7) were included. IOP significantly decreased after surgery (p=0.02). We found a significant pCT, PT and pRNFL thickening from 106.21±30.27µm, 133.21±74.21µm and 58.88±11.27µm before surgery to 130.71±70.66µm, 157.92±112.34µm, and 62.88±9.85µm at one week after surgery respectively (p<0.05). The mean optic disc cupping and LC depth reduced from 358.46±146.61µm and 496.96±136.57µm to 302.33±130.06µm and 477.56±147.81µm respectively (p<0.01). We found a significant correlation between the mean pCT thickening and both IOP lowering and reversal of optic disc cupping ((Spearman rho:0.754, p=0.000; Spearman rho:0.582, p =0.018 respectively).

Conclusions:

Peripapillary choroidal significantly thickened after DS and this change significantly correlated with IOP reduction and optic disc cupping reversal. Larger series with longer follow-up are necessary to set if these changes are reversible and its role in glaucoma evolution.
