Comparison of blebs obtained after microshunt surgery vs routine trabeculectomy with optical coherence tomography

Abstract Details

Purpose:

To classify blebs assessed by anterior segment OCT and photograph in glaucoma patients operated with conventional trabeculectomy and MMC as compared to patients implanted with Microshunts four and five years postoperatively.

Setting:

Glaucoma has become one of the main causes of blindness and an effective, affordable and sustainable treatment is a step towards the right direction for the cure of the disease.

Methods:

Retrospective, comparative and observational study of blebs obtained with conventional trabeculectomy and two different surgical techniques with Micoshunts for the creation of bulla combined with MMC at concentration of 0.4mg/ml applied during three minutes. The OCT images were obtained with radial and transverse scans through the apex of the blebs. These blebs were graded for height, total volume, tangential height and width, thickness of wall, apposition to episclera, cystic compartments, and scleral flap patency. Twenty patients were included in each group at four and five year follow-up.

Results:

Blebs obtained with trabeculectomy and MMC had thinner walls and larger areas. Those created with the Microshunts were spread over a greater area, lower height but greater width.

Conclusions:

The blebs created ab-externo with application of MMC had thinner walls and were more diffuse with lower height than those obtained with other Microshunt devices. The difference is probably due to the application MMC with Merocel sponges under the conjunctiva.
