Purpose:
To compare prospectively between eyes the capsulotomy tear force need to break the explanted capsule and the CDE needed to remove the lens during FLACS and manual cataract surgery.

Clinical trial

Methods:
36 patients were prospectively recruited to have FLACS (Lensx, Alcon) in the worse eye and manual surgery in the better eye. The ocular scatter inder (Accutarget, a surrogate marker for cataract density) and the cumulative dissipated energy needed to explant the lens was measured. Results:
The force needed to tear the capsule extracted after manual surgery was 2.16g± 0.42g (mean ± SEM). There was no significant difference (p>0.99) when compared with FLACS capsulotomy (2.19± 0.38g; mean ± SEM). The CDE required was 15% to 50% lower with FLACS. A larger reduction in CDE was noted for denser cataracts.
Conclusions:
There is no significant difference in the tear strength of the discarded capsule between Lensx FLACS and manual surgery. In denser cataracts there is a marked reduction the amount of CDE required using FLACs for lens removal compared with manual surgery
