Impact of femtosecond laser-assisted cataract surgery (LenS) vs micro incision cataract surgery on early postoperative macular thickness in diabetic patients

Purpose:
Purpose of this study was to compare impact of Femtosecond Laser Assisted Cataract surgery (Alcon LenSx) & Micro Incision Cataract Surgery on Alcon Centurion Vision System with Aspheric Hydrophobic foldable IOL implantation on early post-operative macular thickness in diabetic patients with no evidence of diabetic retinopathy under cover of Topical Pre and post NSAID drops.
Setting:
This is a prospective, non-randomized and comparative case series of two group of patients, who underwent Femtosecond Laser assisted cataract surgery by Alcon, LenSx and MICS on Centurion Vision System respectively by a single surgeon during the period of October 2014 to September 2015 at tertiary eye care centre, India.
Methods:
35 eyes of 35 patients underwent uneventful Cataract Surgery in both study groups: Femtosecond Laser Assisted (Group 1) and Micro Incision Cataract Surgery (Group 2).
Central subfield thickness of all patients was measured by Optovue SD OCT pre-operatively 2 hours before surgery, at 7 days and 45 days after surgery.
Age and Pre-operative macular thickness values were incorporated as covariates to adjust for their effects on post-operative results.
Primary outcomes were Pre-operative, 7th and 45 days Post-operative OCT retinal thickness and change in CSMT at 7th and 45th days post-operatively with respect to Pre-operative retinal thickness values in two groups.
Results:
There were no significant differences between the two groups regarding age (p=0.403), sex (p=0.313), Refractive Error (p=0.502) and effective phacoemulsification time (p=0.88).
Mean of BCVA in both groups were 0.021 ± 0.053 log MAR and 0.034 ± 0.051 log MAR at 45 days post-operatively.
Post-operative macular thickness in both groups showed no significant increase at 7 and 45 days. Mean Preoperative CST was 262.74±13.82 in group 1 and 261.86±15.14 in group 2 (p=0.799), 7 days postoperative CST were 278.94±24.56 & 278.91±24.59 (p=0.996) and 45 days postoperative CST were 272.89±13.68 & 277.32± 23.73 (p=0.342) for group 1 and group 2 respectively.
Conclusions:
With Topical Pre and post NSAID drop coverage, both FLACS and MICS group showed no early Post operative Cystoid macular edema in diabetic population with pre operative normal retina.
No significant difference was found between group 1 and 2 on Post operative macular status and Best Corrected Visual Acuity in diabetic population with pre operative normal retina.
