Intraoperative difficulties with femto laser-assisted cataract surgery (FLACS)

Abstract Details

Purpose:

To document the intraoperative difficulties of Femto-Laser Assisted Cataract Surgery (FLACS), in the first 30 eyes in Magrabi Eye and Ear Hospital, Abu Dhabi, UAE

Setting:

Single center ,Magrabi Eye and Ear Hospital, Abu Dhabi, UAE

Methods:

The study involved the first 30 eyes (20 patients) underwent Femto-Laser Assisted Cataract Surgery (FLACS) by two certified surgeons. The technique utilized the LenSx femto laser system (Alcon) to create femtolaser assisted corneal incisions, performing capsulorhexis , and lens fragmentation. Then OZilĀ® Torsional hand piece of Infiniti machine (Alcon) was used for phacoemulsification and finally intraocular lens was implanted in the capsular bag.

Results:

Docking failure needed unlocking and redocking occurred in 3 eyes (10%), incomplete corneal incision occurred in 4 eyes (13.3%). Torsional eye movement resulted in clockwise shift of corneal incisions in two eyes (6.7%). Premature opening of the main incision during surgical draping occurred in one eye (3.3%) manifested by shallow AC and pupillary shift. Miosis happened in 10 eyes (33.3%). Incomplete capsulorhexis occurred in 3 eyes (10%), and one of them caused anterior capsular tear. Gas bubbles demanded carful hydrodissection in all cases, as well as incomplete divided nuclei. No posterior capsule tear was documented.

Conclusions:

Femto-Laser Assisted Cataract Surgery (FLACS), is a safe procedure, however surgeons are encouraged to pay close attention to avoid possible complications that is inherent to the technique.
