Femtolaser-assisted and conventional phacoemulsification: comparison of visual outcomes in patients with bilateral diffractive IOL implantation

Purpose:
To compare visual outcomes of conventional and femtolaser-assisted cataract surgery with bilateral implantation of diffractive IOL (AT LISA 809M, Carl Zeiss).
Setting:
Branchevsky Eye Clinic , Samara, Russia
Methods:
This comparative analysis of case series included two groups of patients: conventional phacoemulsification (86 eyes) and femtolaser-assisted (with LenSx, Alcon) phacoemulsification (58 eyes) with bilateral implantation of Zeiss AT LISA 809M IOL. The main outcome measures were obtained at 3 months post op and included monocular and binocular distance and near uncorrected and corrected visual acuity (UDVA, CDVA, UNVA, CNVA), objective and subjective spherical and cylindrical components of refraction, spherical equivalent (SE) refraction.
Results:
At 3 months, mean decimal UDVA was 0.87 ± 0.18 (femtolaser-assisted group) and 0.83 ± 0.20 (conventional phaco) (t=0.94), mean CDVA was 0.97 ± 0.11 (femtolaser-assisted group) and 0.96 ± 0.14 (conventional phaco) (t=0.35), mean UNVA was 0.77 ± 0.21 (femtolaser-assisted group) and 0.72 ± 0.24 (conventional phaco) (t=0.98). Mean objective spherical component of refraction was -0.01 ± 0.55 (femtolaser-assisted group) and 0.01 ± 0.62 (conventional phaco) (t=.17), and cylindrical component of refraction was 0.55 ± 0.52 (femtolaser-assisted group) and 0.7 ± 0.57 (conventional phaco) (t=1.33).
Conclusions:
Both technologies - conventional and femtolaser-assisted phacoemulsification with diffractive IOL AT LISA 809M implantation provided excellent distance and near visual outcomes. Monocular UDVA and UNVA appear slightly better in the femtolaser-assisted group. Although differences in refraction predictability in two groups were statistically non-significant, variation of both spherical and cylindrical components of 3 months post op. refraction was wider in conventional phacoemulsification group. All patients in this study achieved complete spectacle independence for far and near visual activities.
