Predictability, efficacy and safety of femtosecond laser-assisted small incision lenticule extraction (SMILE)

Abstract Details

Purpose:

To analyse the results of SMILE in terms of its predictability, efficacy, speed of visual recovery and safety.

Setting:

Centre for Sight, Hyderabad, India - a private tertiary care super-specialty eye hospital.

Methods:

A retrospective analysis of 152 eyes (76 patients, 41 males) of the age group of 19-42 years and having a mean preop spherical equivalent of -3.89 +/- -1.89 diopters and mean cylinder of -1.2 +/- -0.6 diopters, who underwent SMILE procedure for correction of mild to moderate myopia with or without astigmatism. Evaluation included visual acuity, refraction, low contrast acuity, mesopic acuity, corneal topography, tear film function tests, and filling up of subjective response questionnaire preop and 1 week, 1 month, 3 months and 6 months postop.

Results:

136 (89.5%) and 185 (90.68%) eyes were having an unaided visual acuity of 6/6 or better at 1 week and 1 month respectively. No eye lost any line of best corrected acuity. 144 (94.7%) eyes had a best corrected mesopic acuity of 6/6 or better at 1 month while 138 (90.7%) eyes had a low contrast (5%) acuity of 0.1 or better. 11 (14.4%) patients at 1 month and 6 (2.94%) complained of glare at 6 months. 41 (46.4%) eyes showed some increase in the higher order aberrations at 1 month, however mean HOA showed a downward trend from 1 to 6 months. Tear function returned to normal in 128 (84.2%) eyes by 6th month.

Conclusions:

SMILE is a very predictable and effective and safe procedure for correction of mild to moderate myopia and astigmatism. It also provides a very good quality of vision to most of the cases and the induced ocular surface changes revert back to normal by 6 months in majority of the cases.
