Predictive factors for short-term intraocular pressure fluctuations in FLACS

Abstract Details
Purpose:
To assess IOP changes at different time intervals during femtosecond laser pretreatment followed by cataract phacoemulsification and PC IOL implantation.
Setting:
S. Fyodorov Eye Microsurgery Federal State Institution, Moscow, Russia
Methods:
31 patients (34 eyes) with age-related cataracts (grade II-IV) were included in the study. Laser pre-treatment was performed with LensX (Alcon, USA) followed by phacoemulsification with Centurion (Alcon, USA). With the patient in supine position, IOP was measured with Tonopen XL (Tonopen XL, USA) at 3 time points: preoperatively, right after undocking, 15 min later (patient lying on the operating table under the surgical microscope). Preoperatively all patients had conventional eye examination, additionally central corneal thickness registered with anterior segment OCT (Visante, USA). Intraoperative fluidics parameters were recorded, anterior chamber stability and lens fragments followability were assessed with a questionnaire.
Results:
The mean baseline IOP was 15.4±4.5 mm Hg and was elevated up to 18.3±8.7 mm Hg (P = .03) right after undocking of the laser system from the eye. IOP registered after undocking was correlated with age (r = 0.32; P= .091), corneal central thickness (r = -0.97; P=.005). IOP after 15 min. was correlated with anterior chamber depth (r = -0.56; P=.025), cataract grade (r = 0.54; P=.027), axial eye length (r = -0.56; P= .025), total volume of irrigating fluid (r = 0.64; P=.024), and equivalent US energy (r=0.61;P=.034).
Conclusions:
Femtosecond laser (with LensX and Soft fit interface) induces minimal IOP fluctuations. Age, cataract density, decreased central corneal thickness are predictive factors for IOP rise in patients with age related cataracts.
