Effect of changes in power settings on the refractive effect of SMILE surgery

Abstract Details
Purpose:
Comparison of various energy parameters in use of Smile technology and evaluation of clinical efficacy in myopia correction.
Setting:
Irkutsk Branch of S. Fyodorov Eye Microsurgery Federal State Institution, Irkutsk, Russia
Methods:
Study included 36 patients (72 eyes) operated by Smile method. Patients were divided into 2 groups. First group included 21 patients operated by standard mode (140 nJ, spot distance 3.0 μm). Second group included 15 patients operated by quick mode (180 nJ, spot distance 4.5μm). Myopia in all cases was mild to moderate, in 52% it was combined with astigmatism. Uncorrected visual acuity before surgery in both groups was comparable. Standard ocular examination was made before and after surgery with multiplicity 1 day and 1 month. Status of the cornea was assessed on the scan of anterior segment «Pentacam».
Results:
During surgery and in postoperative period the complications weren’t observed, separation of tissue before lenticular extraction did not cause trouble in both groups.
But on the day after surgery in patients of second group the monocular uncorrected distance visual acuity was significantly higher 0,96±0,006 compared with patients with a standard mode - 0,81±0,05 (p≤0.05).
At first day spherical component in patients of compared groups also differed after surgery. Patients of first group had spherical component 0,19±0,36, cylindrical one 0,48±0,2, in second group 0,05±0,2 diopters, 0,51±0,1 diopters respectively.
Study of refractive characteristics and corneal aberrations using Pentacam revealed no significant differences.
Conclusions:
The use of quick mode (180 nJ, spot distance 4.5 μm) allows achieving a high refractive results already in the first day after surgery with less energy impact on corneal tissue
