Measuring corneal haze with OCT after epithelial disruption corneal collagen cross-linking with trans-epithelial riboflavin in keratoconus

Abstract Details
Purpose:
To quantitatively measure CXL (Corneal Collagen Crosslinking) associated corneal haze by Spectral Domain OCT in patients with keratoconus undergoing CXL by epithelial disruption with Riboflavin TE (0.25%).

Methods:
Retrospective, observational, case review of 14 eyes of 13 patients with keratoconus, who underwent CXL with epithelial disruption and Riboflavin TE (0.25%) were analyzed by serial post-operative Anterior Segment Spectral domain OCT (preoperatively, postoperative day 1, 1 month, 3 months and 6 months) and Sirius Tomography (maximum keratometry-Kmax measured preoperatively, postoperative 1 month, 3 month, 6 month and 1 year) and specular microscopy (preoperatively, postoperative 6 months and 1 year). Clinical correlation of haze on OCT was done with the haze on slit-lamp examination on each examination.
Results:
On first postoperative day increase in pachymetry on OCT by mean- 72.58 microns (range 20-181 microns) was seen. Diffuse central haze with mean depth 368.83 microns (range 217-485 microns), tapering in periphery with variable depths was documented on OCT. Compaction of corneal stroma over post-operative 1-12 months with compaction of haze compared to postoperative day 1 is observed on OCT. Mean difference of depth of haze from postoperative day 1 to 6 months was 105.85 microns (range 17-180 microns). Stromal thickness returned to preoperative values approximately at 3 months, depth and density of haze had gradually reduced from 3-12 months.
Conclusions:
Corneal haze after CXL is considered an indirect measure of the depth of crosslinking and it can be quantitatively measured on OCT and shows initial increase in corneal thickness followed by gradual compaction and decrease in haze over a period of 12 months in eyes undergoing CXL with epithelial disruption with Riboflavin TE (0.25%). These eyes remained stable as observed in terms of visual acuity and topographic K-max values suggesting no progression of keratoconus.
