Corneal cross-linking WA (window absorption) in the treatment of severe corneal ulcer: a long-term follow-up

Abstract Details

Purpose:

To report a long term follow-up about safety and efficacy of treatment of severe infectious corneal ulcers with Corneal cross-linking WA (Window Absorption).

Setting:

Pietro Rosetta M.D. Riccardo Vinciguerra M.D. Emanuela Legrottaglie M.D. Paolo Vinciguerra M.D. Humanitas Research Hospital Milan (Italy)

Methods:

9 eyes with severe keratitis and associated corneal melting (3 Pseudomonas aeruginosa, 2 Staphylococcus Epidermidis, 1 Streptococcus pneumonia, 1 Acanthamoeba, 2 negative antibiogram) were treated with Cross-linking window absorption (CXL-WA). CXL-WA was always performed as an adjunct to systemic and topical antibiotic therapy. In all treatments the UV-A light irradiance was 3 mW/cm2 or 5.4 J/ cm2 and the exposure lasted for 30 minutes. All patients were examined after a minimum follow-up of one year. Anterior segment OCT was always performed after treatment to measure the demarcation line depth.

Results:

In all cases, CXL halted the progression of the ulcer. After a minimum follow-up of one year, none of the patients had recurrence of the infection, OCT demarcation line was in all cases >200 microns.

Conclusions:

CXL-WA is a confirmed technique as an adjunct to antibiotic therapy of treatment of infectious corneal ulcers, delaying or preventing more invasive approaches.
