Abstract Details
Purpose:
To determine the efficacy of corneal crosslinking and Boston lens in improvement of visual acuity of keratoconus patients
Setting:
Modern Eye Hospital & Research centre, Nellore, Andhra Pradesh, India
Methods:
Prospective , Observational & Non-comparitive study of 40 eyes of 20 keratoconus patients .Patients underwent Uncorrected Distance Visual Acuity(UCVA), BestCorrected distance visual acuity(BCVA), subjective refraction, spherical equivalent estimation, topography, Pachymetry & Fundus evaluation. Patients not improving with glasses underwent Boston lens trial . Later patients underwent collagen cross linking & patients not improving with Boston lens underwent Deep Anterior Lamellar keratoplasty (DALK) Or Penetrating keratoplasty (PKP).Post operative Patients underwent Uncorrected Distance Visual Acuity, Corrected distance visual acuity, subjective refraction, spherical equivalent estimation, topography & Pachymetry
Results:
At 3 years follow up mean spherical equivalent reduction was noted to reduce by 0.5D,UCVA improved by 0.5. to 1.0 line on an average , BSVA improved by 3.6 lines with Boston lens in 75%( i.e 30 eyes )10%( 4 eyes) of40 patients improved with glasses alone & 15%( 6 eyes) of total 20 patients(40 eyes) required DALK/ PKP
Conclusions:
CXL is safe & effective , especially when combined with Boston lens in keratoconus patients thus avoiding major surgical intervention procedures in patients not improving with glasses
