Corneal cross-linking in extremely thin keratoconic eyes: feasibility study of using contact lenses and collagen shields

Abstract Details
Purpose:
To determine the intrinsic properties and characteristics of contact lenses and collagen shields fitting on keratoconic eyes in preparation for future corneal cross linking in sub 400um corneas
Setting:
Birmingham and Midlands Eye Centre.
Methods:
Measurements of the anterior segment were made with a keratoconic volunteer wearing contact lenses (Proclear, Focus Dailies and Bausch and Lomb Softlens 38) and collagen shields (Kestrel 24 and 72) unsoaked and soaked in riboflavin. The tear film and central contact lens/collagen shield thickness were measured using a Heidelberg SL OCT.
Results:
All the devices had differing tear film and central thickness profiles. Bausch and Lomb Softlens 38 was the thinnest when comparing purely device thickness, as well as when summing device thickness plus tear film and endothelium to anterior surface of the device.
Conclusions:
The viability of performing contact lens assisted corneal cross-linking in patients excluded the technique due to corneas thinner than the recommended cut off, has been previously demonstrated. This data will further enhance the feasibility of treatment in similar cases and possibly allow customisation of collagen cross linking according to the thickness of the cornea.
