Evaluation of iontophoretic collagen cross-linking for early stages of progressive keratoconus comparing to standard cross-linking: 24 months results

Abstract Details

Purpose:

To evaluate the efficacy and safety of iontophoretic collagen crosslinking(I-CXL) comparing to standard epithelium-off collagen crosslinking (CXL) in early stages of progressive keratoconus.

Setting:

Oftaprof Clinic, Iasi, Romania

Methods:

The study included 40 eyes of 40 patients with progressive keratoconus that underwent iontophoretic CXL and the results were compared to a group of 40 eyes from 40 patients that received standard CXL. The follow-up period was 24 months. All patients were evaluated for uncorrected (UCVA) and corrected (CDVA) distance visual acuities, corneal topography and pachymetry.

Results:

The mean patient age was was 26.53±3.77 in standard CXL group and 28.33±4.01 in I-CXL group. The mean UCVA and CDVA significantly improved at 12,18 and 24 months in both groups.At 24 months the difference between the changes in UDVA and CDVA in both groups was not statistically significant (P>0.05).A statistically significant decrease in the manifest sphere and cylinder value was observed at the 24 months follow-up visit;the change was significantly greater in the epi-off group (P<0.05). The epi-off CXL group had greater reduction in Kmax values than the I-CXL group with statistical significance at 12, 18 and 24 months( P<0.01, and P<0.001, respectively).

Conclusions:

The results of the study indicate that all parameters improved or remained unchanged after iontophoretic collagen crosslinking intervention. When comparing the results between the two methods, I-CXL was as effective as the standard technique. Our results show that iontophoretic CXL has the potential to stop the progression of keratoconus.
