Abstract Details

Purpose:

To report initial clinical results of diluted alcohol and ionthophoresis assisted transepithelial corneal collagen cross-linking (DAI-CXL).

Setting:

Gazi University, School of Medicine, Department of Ophthalmology, Ankara, TURKEY

Methods:

Forty-two eyes of 32 patients who have diagnosed progressive keratoconus and underwent DAI-CXL were included in this prospective nonrandomized interventional study In this new tecnique 10 % ethanol solution was applied for 10 seconds at the beginning of the treatment then standart iontophoresis CXL was performed. Uncorrected visual acuity (UCVA), best corrected visual acuity (BCVA), Scheimpflug tomography, anterior segment optical coherence tomography (AS-OCT), and in vivo confocal microscopy (IVCM) findings were recorded at baseline, first week, and 1, 3, 6, and 12 months postoperatively.

Results:

The mean follow-up was 9.53±1.5 month, and a non-significant trend toward to improvement was detected in BCVA at the end of the follow up period (p=0.06). Corneal topography and Scheimpflug tomography parameters were significantly less then base line (p=0.0001). The corneal stromal demarcation line was clearly identified in all cases, the mean depth was 306,2±52,4 μm. IVCM findings were very similiar with the standart method.

Conclusions:

The result of the study, suggests that dilute ethanole application and the change in total UVA energy dosage (7.2 J/cm2) and riboflavin concentration (0.2%), can enhance the effect of I-CXL, without inducing corneal epithelial injury. Our results also indicate the efficacy of DAI-CXL in stabilizing the progression of keratoconus. Moreover AS-OCT and IVCM findings suggest that the efficacy of DAI-CXL seems superior then the other reported transepithelial methods.
