Short-term results of contact lens-assisted collagen cross-linking in thin corneas with keratoconus

Purpose:
To evaluate the short term results of contact lens assisted collagen cross linking (CLA-CXL) in keratoconic eyes with thin corneas
Setting:

Methods:
Progressive keratoconus patients with a minimum corneal thickness of <435µ underwent accelerated CLA-CXL (9 mW/cm2 UV-A for 10 minutes) with precorneal UV-filter free comfilcon A silicone hydrogel contact lens (Biofinity®, Cooper Vision) pre-soaked into 0.1% riboflavin – 1.1% HPMC solution without dextran (MedioCrossM®, Avedro, Inc). Patients had full ophthalmological examination including topography with Scheimpflug camera (Pentacam®, Oculus) and anterior segment optical coherence tomography (VisanteTM AS-OCT, Zeiss). The changes in uncorrected and best spectacle-corrected visual acuities (UCVA and BSCVA, respectively), simulated keratometric (SimK) readings, as well as intrastromal demarcation line depths were analyzed
Results:
Five eyes of 5 patients were included into the study. Three eyes had primary CLA-CXL; 2 eyes had intracorneal ring segment implanted at least 1 month before. Patients were followed by a median of 1.5 months (range:1-2.5) Mean UCVA was 0.25±0.17 preoperatively; 0.35±0.22 postoperatively (p=0.22). Mean BSCVA was 0.44±0.13 preoperatively; 0.62±0.08 postoperatively (p=0.017). Mean SimKaverage was 49.92±2.98D preoperatively, 50.06±2.98D postoperatively (p=0.94). Mean thinnest pachymetric reading was 425±9.07µ preoperatively and 414±43.02µ postoperatively (p=0.57). Postoperative AS-OCT imaging revealed a stromal demarcation line at a depth of 246.7±72.02µ. None of the eyes revealed clinical signs of endothelial cell damage and stromal edema postoperatively
Conclusions:
CLA-CXL might be considered as an alternative method for safe collagen cross-linking in thin corneas. In order to better assess safety and efficacy, specular microscopy examinations and long term follow-up in terms of disease progression are warrented.
