Treating progressive keratoconus using VICTUS femtosecond laser platform and accelerated intrastromal cross-linking

Abstract Details
Purpose:
To describe a novel surgical technique for the management of progressive keratoconus using VICTUS Femtosecond Laser Platform and accelerated intrastromal cross-linking with Dextran Free Riboflavin
Setting:
To perform an accelerated corneal intrastromal collagen cross-linking without any epithelial debridement
Methods:
We treated a total amount of six patients, of which five have a progressive keratoconus diagnose and the other a progressive post-LASIK ectasia. For all of them we followed the same protocol. In the first place, a corneal pocket at 140 micrometers depth was created using VICTUS Femtosecond Laser Platform. Subsequently, Dextran Free Riboflavin (VibeX Xtra TM) injected into this pocket so as to perform an accelerated corneal intrastromal collagen cross-linking (Avedro KXL Protocol) without any epithelial debridement. The total amount of energy used was 7,2 J/cm2 and total UV power was 30 mW/cm2 during 4 minutes time.
Results:
The cornea remained clear in all our patients while the central keratometry was reduced. An improvement in uncorrected visual acuity was recorded in these patients over the last six months, even though they are all still under strict control of their evolution.
Conclusions:
The use of Femtosecond Laser-created pocket combined with an accelerated corneal intrastromal collagen crosslinking using Dextran Free Riboflavin is a safe technique for the management of progressive keratoconus; and showed up to be effective in terms of improving uncorrected visual acuity.
