Purpose:
To describe a surgical approach of a modified DSAEK procedure in a case with tube Baerveldt glaucoma implant
Setting:
Sussex Eye Hospital Brighton and Sussex University Hospitals. NHS Foundation Trust. Eastern Rd, Brighton, East Sussex BN2 5BF United Kingdom
Methods:
Case report. Surgical Technique. A 72 year-old patient with an only eye with corneal decompensation secondary Baerveldt glaucoma tube implant in the anterior chamber. His visual acuity was 6/36 and he underwent a double procedure including tube repositioning and modified DSAEK
Results:
Initial glaucoma tube withdrawal and shortening followed by a modified DSAEK using a 7.5mm donor graft in which a peripheral notch was created. The graft was positioned in a way that the notch was overlying the glaucoma tube. This customization of the DSAEK was done to avoid future erosion of endothelial cells from the DSAEK which may threaten decompensation and graft failure. There were no intraoperative and neither postoperative complications, his endothelial graft remains well functioning and his visual acuity has improved to 6/18 after 5 months
Conclusions:
This modified technique of DSAEK with a notch offers the benefits of potentially reducing the future endothelial cell loss from the transplant and hence reduces the risks of graft failure because the tube is not in close proximity of the DSAEK tissue
