Outcomes of femto-assisted deep anterior lamellar keratoplasty for keratoconus: a report on 214 consecutive cases

Purpose:
To report outcomes of femto-assisted Deep Anterior Lamellar Keratoplasty (femto-DALK) in a large series of patients with keratoconus
Setting:
Single-center, retrospective interventional case series
Methods:
Femto-DALK was performed using original manual scleral tunnel technique of Melles. Best spectacle corrected visual acuity (BSCVA), spherical equivalent refraction (SE), keratometric readings, and specular microscopy data were recorded before and at regular follow-ups after surgery. Paired-Samples T-test was used to compare pre- vs. post-operative data
Results:
Mean age was 28 years , and 135 were male. Rim shape was mushroom in 135 eyes, polygonal in 76, and round in 3. The logMAR BSCVA was improved from 0.65 before operation to 0.17 at 6 months. The SE refraction was improved from -5.55D to -2.29, mean keratometry value from 55.00 to 44.81, keratometric astigmatism from 5.15 to 2.68, endothelial cell density from 2412 to 2630 cells/mm2. Decentration (2.3%), suction loss (1.4%); Descemet’s membrane perforation (0 %). Postoperative complications comprised persistent epithelial defect (1.4%), epithelial rejection (2.8%), stromal rejection (1.4%), epithelial herpetic keratitis (0.5%), persistent elevation of IOP (1.4%).
Conclusions:
Femto-DALK was a successful method for improving BSCVA and restoring corneal anatomy in patients with keratoconus, with low rate of complications.
