Surgical outcomes in chronic ocular GVHD

Abstract Details

Purpose:

To asses the ocular complications that can appear in chronic ocular GVHD and its need of surgical techniques.

Setting:

DED is the most common manifestation of chronic GVHD and can led to great impact on morbidity. We present a case clasified as mild/moderate chronic ocular GVHD that appeared with bilateral corneal melting that needed surgical procedures.

Methods:

We report a case of ocular chronic GVHD diagnosed and clasified as mild/moderate with GVHD consensus group test and severity index. Before evaluation and initial good response to treatment, the patient appear at urgency room with poor clinic manifestations. But at slit lamp examination it reveal a corneal perforation on right eye that needed conjunctival graft. We closely follow up the patient and we found a corneal melting on his left eye a month later that needed contact lens, systemic and topical treatment.

Results:

Surgical outcome following conjunctival graft on right eye was adecuate, reachingvisual acutity of 0.6. On the left eye the corneal melting resolved without surgery.

Conclusions:

Spontaneous corneal ulceration can occur on DES in GVHD patients without great clinical manifestations due to neurotrophy. Close follow up of the patients must be made.
