LASIK-flap loosening due to Fuchs endothelial dystrophy reversed by Descemet's membrane endothelial keratoplasty

Abstract Details

Purpose:

A 58-year-old male patient presented with bilateral visual impairment for Fuchs endothelial dystrophy (FED) and visually significant cataract after laser in situ- keratomileusis (LASIK).

Setting:

Netherlands Institute for Innovative Ocular Surgery/ tertiary referral center

Methods:

Phacoemulsification and intraocular lens implantation was carried out but visual acuity did not improve sufficiently due to an increase in central corneal thickness and the presence of a fine cleft with interface fluid between the LASIK flap and the residual stromal bed as depicted on anterior segment optical coherence tomography.

Results:

After uneventful Descemet's membrane endothelial keratoplasty (DMEK) the interface fluid increased initially but resolved spontaneously within a week while visual acuity improved rapidly.

Conclusions:

This case demonstrates that a central LASIK-flap detachment resulting from FED may be reversible by DMEK with fast visual recovery.
