Is there any relationship between the fluid temperature and the unfolding time of Descemet membrane roll?

Abstract Details

Purpose:

To investigate whether unfolding time of the cornea descemet membrane endothelial keratoplasty (DMEK) graft roll formations changing in various fluid temperatures.

Setting:

This study was prospective, ex vivo and experimental. The study was conducted between June, 2014 and June, 2015 with 16 donor corneas which were taken from Eye Bank of Adana Numune Training and Research Hospital and were not appropriate for use due to positive serological tests (HbsAg+ or antiHCV+).

Methods:

In the present study which investigated whether there is a relationship of descemet membrane (DM) roll unfolding time with balanced salt solution (BSS) temperature used, the tissues which were prepared under operating room conditions and surgery microscope were stained with trypan blue solution. Using tying forceps, each tissue was put into glass containers having different temperatures. Whether there were any changes in roll formations was observed and photographed at the beginning, at the 5th minute, and at the 10th minute under microscope.

Results:

DM roll formations which are prepared can vary according to the donor cornea received. While some of them form a tighter roll, some others can form a more open roll. At the beginning, there was not a statistical difference in the groups between the DM roll formations obtained in the 23°C BSS temperature in terms of grade (p=0,273). In all groups, no differences were observed in the DM rolls put into different BBS temperatures at the 5th and 10th minutes. In all groups, neither tightening nor opening was observed in DM roll formations.

Conclusions:

Different BSS temperatures in this study were found to have no effects on DM unfolding time. Methods different than BSS temperatures which are less traumatic and make DM unfolding easier should be investigated with a view to reducing endothelium cell loss.
