Purpose:
To report and compare the survival rate, visual outcomes, complications and treatments of repeat penetrating keratoplasty (RePK), Descemet stripping endothelial keratoplasty (DSAEK), Descemet's membrane endothelial keratoplasty (DMEK) after failed penetrating keratoplasty (PK)
Setting:
La Mancha Centro Hospital - Alcazar de San Juan, Ciudad Real ,Spain
Methods:
A retrospective comparison of consecutive eyes which underwent: DMEK 10 eyes, DSAEK 7 eyes and 10 RePK, after failed PK. The keratoplasties were realized by a single surgeon.
Data of visual outcomes, complications, immunological reaction, treatments and survival rate were collected. Outcome data were compared between groups.
Results:
DMEK after failed penetrating keratoplasty had the lower rate of immunological reactions and these eyes were treated with topical medications unlike DSAEK and RePK which needed systemic treatment.
DMEK and DSAEK reduced complications related to suture. The patients with DMEK achieved higher visual outcomes than DSAEK and RePK in failed penetrating keratoplasty.
Conclusions:
In our study the DMEK is the better surgical treatment for failed penetrating keratoplasty. Rejection episode before PK failure is an important high risk factor of rejection in subsequent keratoplasties.
