Intraseg® for correction of inferior paracentral keratoconus combined with simultaneous flash cross-linking, refractive and keratometics outcomes

Abstract Details

Purpose:

To evaluate the outcomes of intrastromal corneal ring segment (ICRs) implantation for the treatment of keratoconus assisted with Femtosecond for tunel creation with simultaneous Flash Cross-Linking.

Setting:

This retrospective, observational study on patients who had undergone ICRs and simultaneous cross linking. All surgeries were performed at Zaldivar Institute (Mendoza, Argentina)

Methods:

The study included consecutives eyes of patients with previous diagnostic of keratoconus identify as both clown and junctional pattern from Rabinowitz patterns that underwent ICRs implantation and at least one year follow up. Patients were implanted with INTRASEG® ICRs (Gamma Vision) using a femtosecond laser (IntraLase) for tunnel creation. Uncorrected Distance visual acuity (UDVA), best-corrected distance visual acuity (BDVA), refractive and corneal keratometry were measured preoperatively and at 1, 3, 6, and 12 months postoperatively. Spps for Windows was used for the statistical analysis.

Results:

157 eyes of 123 patients were analyzed, 35 patients underwent bilateral surgery and the rest were operated in just one eye. The mean age of patients was 34.45 years SD 10.4 (14-67), 50,6% were female. The mean UDVA (logMAR) changed from 1.21 ± 0.64 preoperatively to 0,63 ± 0,66 postoperatively (p<0.001), and CDVA from 0.36 ± 0.28 to 0,1 ±0,18(p<.0001). Mean depth of implantation was 389±17.89 (range 340-400). Mean Kmax decreased from 48.16 ± 4.4 (41-61D) preoperatively to 45.9 ± 2.4 (41-53D) (p<.0003) postoperatively, while Kmin changed from 45.90 D to 43.58 D. Mean cylinder decreased from -3.76 (range -1.50 to -8.0) to -2,4 (range -0,50 to -5,75).

Conclusions:

Femtosecond-assisted Intraseg implantation is a promising option for the management of this patterns of keratoconus combined with flash cross linking, reducing topographic and refractive astigmatism and improving both UDVA and CDVA, we consider this is an effective and safe technique without complication rate in our case serie, the results were statistically significant for all cases.
