Anterior segment optical coherence tomography of post-DSAEK eyes to evaluate graft regularity

Purpose:
To assess the relationship between graft thickness and graft regularity following Descemet’s Stripping Automated Endothelial Keratoplasty
Setting:
Ophthalmology Department, Villa Igea Private Hospital, Forli, Italy
Methods:
Anterior segment optical coherence tomography (AS-OCT) images obtained 3 to 6 months following Descemet’s Stripping Automated Endothelial Keratoplasty (DSAEK) were analyzed. Elevation maps and pachymetric maps of the host cornea and donor tissue were analyzed. Graft regularity was determined both by the root mean square of the corneal elevation at its posterior and interface surfaces (RMS-Ep, RMS-Ei), and by mean and standard deviation of pachymetric measurements taken 3 and 6 mm from the corneal center. These parameters were compared between eyes with central graft thickness (CGT) ≥100µm (group 1) and eyes with CGT <100µm (group 2).
Results:
Forty-six eyes of 42 patients with no significant co-morbidities were included. There were 20 eyes (CGT=145.5±40.1µm) in group 1 and 26 eyes (CGT=73.6±15.2µm) in group 2. RMS-Ep was 15.9±6.0 in group 1 and 11.3±4.2 in group 2 (p=.002). RMS-Ei was 10.2±3.3 in group 1 and 9.0±2.6 in group 2 (p=0.09). Standard deviations of graft pachymetry at 3 mm zone deferred statistically between the two groups (p=.009). At 6 mm zone there was no statistically deference between the two groups in terms of graft regularity
Conclusions:
AS-OCT including both topographic and pachymetric analysis can evaluate DSAEK graft regularity. Based on our results, grafts with CGT <100µm demonstrated higher regularity in the 3 mm optical zone in comparison to grafts ≥100µm
