Surgery of the ocular dioptre: solving a keratoconus

Purpose:
Description of a case of bilateral keratoconus, each eye at different stages in the Amsler-Krumeich scale, that was solved with both a therapeutic and refractive approach. The patient was a 36 years old male with keratoconus (KC) grade II in the right eye and grade IV (Amsler-Krumeich scale) in the left eye.

Instituto Universitario Fernández-Vega (Oviedo). Spain.
Methods:
We decided to implant intracorneal ring segments (ICRS) in order to regularize the cornea in the right eye and then implanted a collamer epicristalinian lens to correct the residual refraction. Due to the advanced stage of the keratoconus in the left eye we decided to perform a deep anterior lamelar keratoplasty, combined with lensectomy and later ICRS to treat the residual refraction.
Results:
Preoperatively, the right eye had -15.00 dioptres (D) of sphere and refractive cylinder of -9.00 D. The left eye had -16.00 D of sphere and refractive cylinder of –10.00 D. Distance corrected visual acuity (DCVA) was 0.3 and 0.2 (decimal scale) in right and left eye respectively. Postoperatively, the right eye, after implanting the ICRS got a refraction of 100º–2.50–3.00 and DCVA had improved to 0.4. After suture removal the left eye got 150º – 6.00 and after ICRS implant it was reduced to 160º – 3.00 with a DCVA of 0.6
Conclusions:
ICRS have a double aspect, therapeutic, as they help to regularize the cornea, and refractive, as they can be used on regular corneas to reduce astigmatism, allowing a double approach to the treatment of keratoconus.
