Surgical management of corneal endothelial decompensation following athalamia after trabeculectomy and phacoemulsification: case report

Abstract Details

Purpose:

To describe the management of corneal endothelial decompensation in a case of extensive iridocorneal synechiae due to prolonged athalamia after trabeculectomy and phacoemulsification.

Setting:

First Department of Ophthalmology, National and Kapodistrian University of Athens, Athens, Greece

Methods:

A 71-year-old man presented with bullous keratopathy after trabeculectomy and phacoemulsification. Due to extensive central synechiae, no endothelial keratoplasty was feasible. Visual acuity (BCVA) was reduced to “hand movement”. Viscoelastic-assisted synechiolysis was performed leading to partial reformation of the anterior chamber. Subsequently, descemetorhexis combined with synehiectomy was performed in order to achieve total synechiolysis and proceed with DSAEK.

Results:

Successful DSAEK led to visualization of a fibrous membrane at the level of the pupil osbtructing the visual axis (BCVA was “counting fingers”). Removal of that membrane 1 month after DSAEK led to a rise of BCVA to 0.5.

Conclusions:

The presented case illustrates a staged approach to an eye with extensive anterior synechiae which allowed the performance of endothelial keratoplasty with a satisfactory visual result from hand movement to BCVA 0.5.
