Outcomes of modified mushroom femtosecond laser deep anterior lamellar keratoplasty

Abstract Details
Purpose:
To evaluate the visual, refractive and keratometric outcomes of patients undergoing femtosecond laser deep anterior lamellar keratoplasty (FSDALK) using a modified mushroom geometric pattern.
Setting:
Centre for Sight, London, East Grinstead and Oxshott, UK :Private ambulatory Surgical Centre
Methods:
Retrospective evaluation of 77 eyes of 68 patients who underwent FSDALK. Measures included uncorrected (UDVA) and best corrected visual acuity (CDVA), keratometric and refractive astigmatism, spherical equivalent (CE) and any adverse events.
Results:
52 recipients were male and 25 female. Indications included keratoconus (n=68) and scarring(n=4), Post Lasik ectasia(n=2) corneal dystrophy (n=2). Trephination was performed with the Intralase iFS femtosecond laser and lamellar dissection performed manually (Big bubble attempted in 52, accomplished in 29 and remainder modified Melles).
Mean UDVA LogMAR was 0.70 at 3M, 0.68 at 6 months and 0.66 at 12M postop. Mean CDVA LogMAR was 0.40 at 3M, 0.39 at 6M and 0.35 (20/30) at 1 year. At 1Y 61.9% had CDVA of 20/40 or better. Mean keratometric cylinder at 6 months was 4.61D (SD 2.57) and refractive cylinder 4.13D (SD 2.14). Mean SE at 6M was +0.89D (SD 4.14). Mean time for suture removal was 14.2 Months (SD 6.08).
Conclusions:
FSDALK with an anteriorly undermined modified mushroom technique combined with manual posterior lamellar dissection is a safe and promising technique which results in good visual rehabilitation
