Single-step transepithelial ASLA for myopic patients with recurrent corneal erosion

Purpose:
To evaluate the clinical and refractive outcome of single-step modified transepithelial photorefractive keratectomy (trans-PRK) termed a SCHWIND all surface laser ablation (ASLA) in management of myopic patients with recalcitrant recurrent corneal erosion (RCE) in a prospective non-comparative interventional case series.

New Mowasat Hospital, Kuwait
Methods:
Twenty eyes of 16 myopic patients (12 men, 4 women) with RCE were included. Mean patient age was 26.5 ± 5.5 years (range: 19 to 45 years). All patients underwent ASLA with the SCHWIND Amaris excimer laser system using aspheric ablation profile at a central 7.5 mm ablation zone. Follow up scheduled to 1, 3, 6 and 12 months post treatment. Outcomes measured were pre- and postoperative manifest refraction, spherical equivalent (SE), topographic (corneal) astigmatism, uncorrected (UCVA) and best-corrected distance visual acuity (BCVA), slit lamp evidence of corneal complications and patient satisfaction. All patients completed One year follow up.
Results:
Complete alleviation of RCE symptoms was demonstrated and there was no recurrence of corneal erosion throughout the follow up period.
The mean UCVA improved from 6/60 preoperatively to 6/9 one year postoperatively. The average gain in lines for UCVA was 5.50.
Prior to re-treatment the mean manifest spherical equivalent was –3.25 D ± 1.42 diopter (D) (range from - 0.75 to -5.63). At 1 year follow up 85% of the eyes were within ± 0.50 D SE and 100% were within ± 1.00 D SE.
Conclusions:
ASLA technique using Schwind Amaris Excimer laser was a safe and effective procedure in management of myopic patients with recurrent corneal erosion resulting in the alleviation of symptoms and prevention of recurrences of corneal erosion as well as in improving the visual and refractive outcome in these patients.
