Corneal recurrent ulcer treated with topical matrix regenerating agents (RGTA)

Abstract Details
Purpose:
To report the outcomes of a corneal recurrent ulcer treated with topical matrix regenerating agents (RGTA) after several failed attempts of medical and surgical treatment in a 10 years old patient. The response were evaluated and followed with anterior segment optical coherence tomography (OCT).
Setting:
Department of Ophthalmology at Consorci Sanitary de Terrassa (Barcelona, Spain).
Methods:
A 10-year old patient was referred to our hospital with the diagnosis of a left eye recurrent ulcer that has been treated with topical antibiotics and autologous serum in the last year, with short time improvement before it returns to appear.
Amniotic membrane transplantation was performed in order to treat the deep corneal ulcer. Several anterior segment OCT have shown corneal improvement
One year after the recovery the patient was referred to our cornea department with a new episode of the recurrent ulcer. We treated this new episode with RGTA (CACICOLĀ®) . The response was evaluated with anterior segment OCT.
Results:
After 1 week of treatment the stroma was repeared and 4 weeks after the OCT revealed a complete recovery of the corneal structure.
In the following 9 months the cornea still remains stable.
Conclusions:
Topical regenerating agents interact with components of the extracellular matrix, binding matrix proteins and protecting them from proteolysis, restoring the matrix environment, and improving tissue healing.
In our case this new treatment with RGTA drops provides us a non surgical option for treatment of chronic ulcers, that allows to reserve other options such as tarsorrhaphy, conjunctival flaps, AMT, and lamellar or PK for a chronic ulcer resistant to medical treatment, specially in a 10 Years old patient.
