Anterior segment parameters evaluation using Pentacam during valsalva maneuver in keratoconus patients

Abstract Details
Purpose:
The study aims to explore the effects of the valsalva maneuver (VM) on ectatic cornea during the corneal topography scan with Scheimpflug camera.
Setting:
This prospective observational study involved 100 eyes of 50 patients who had bilateral keratoconus (KC).
Methods:
Corneal topography was first performed when the patients were in resting position, immediately afterwards it was repeated in the course of VM. Topography images were taken using Pentacam (Oculus Optikgerate, Wetzlar, Germany) which enables to get images of the anterior and posterior surface of the cornea, the iris, and the anterior and posterior surface of the lens in a movable virtual eye
Results:
Average age of the participants who had KC in both eyes was found 24.14±6.59 years. Of the eyes included in the study, 7% was stage 1 KC, 47% was stage 2 KC, 32% was stage 3 KC, and 14% was stage 4 KC. Indices of KC were not affected by the VM significantly. Similarly, no statistically significant effects were found on stage of KC, and pachymetric progression index average during VM. Pupil dimeter (PD) displayed a statistically significant increase during VM (p=0.017). On the other hand, there was a statistically significant decrease in the angle value during VM (p=0.001).
Conclusions:
No changes were found in KC indices and stage of the disease with VM. Besides, it was found that there was an increase in PD and a decrease in angle value; and these changes displayed similarity with studies on normal corneas.
