Severe corneal abscess by pseudomona aeruginosa in a baby of 5 months old

Abstract Details

Purpose:

The pseudomonas keratitis are associated with the use of contact lenses or trauma, while infections in children are usually caused by Gram + . We report the clinical case of a baby of 5 months old with severe corneal infection. No illness. Mother denies previous trauma.Microbiological study revealed pseudomonas aeruginosa.

Setting:

Clinical study by slit lamp, ocular and orbital ultrasound and microbiological study of corneal scraping

Methods:

Clinical case. Baby, 5 months old.Emergency admission for red eye 2 days of evolution. Mother denies previous trauma. Exploration : Pre-septum cellulitis and corneal abscess with stromal thinning that prevented display of the anterior pole. No foreign bodies. Ultrasound: no intraocular or orbital affectation. Corneal scrapping is performed and immediately is treated by enhanced vancomicyn and ceftazidime hourly. The microbiological study reveals pseudomonas aeruginosa sensitive to cephalosporins and fluorquinolones.

Results:

The stromal infiltrate improves with the prescribed treatment and continue with quinolone until resolution of the same As a sequel is a central walleye. It is planned to make a penetrating keratoplasty or DALK, but the family prefers not to perform surgery. Currently developing a sensory deprivation strabismus

Conclusions:

Pseudomona aeruginosa keratitis in children raises the question about the best treatment option Medical approach:we consider at the first option third generation of cephalosporines . It´s essential to take a sample of the abscess to analyze microbiological origin. Surgical treatment: it´s very important the valuation of complications and functional recovery. The family didn´t give us the option to do surgery, but thought it would be feasible to perform DALK in order to achieve the best possible visual acuity.
