A new method of implantation of lens capsular bag (CB) expander: preliminary results

Abstract Details

Purpose:

Analysis of the results of joint endocapsular implantation of two open-bag devices (capsular bag expander (CBE) and plate-torsion haptic intraocular lens (PTHIOL) based on the concept of torsion haptic.

Setting:

Department of Ophthalmology, Penza Federal State Institute for Postgraduate Medical Studies of the Ministry of Health of Russian Federation, Penza, Russia.

Methods:

CBE as a flexible, transparent hydrophobic acrylic plate 5.5x15.5x0.2 mm ('Reper-NN', Ltd.) with two haptics evenly stretching CB in the equatorial zone and anteroposterior direction was used. Sequential implantation of PTHIOL and CBE was made on 5 eyes of 5 patients with cataract and myopia after uncomplicated PHACO through 2.2 mm incision in the perpendicular meridians with the placement of the one on the posterior and the other on the anterior capsule. Determining of the native lens and the CB with implants parameters was performed before and after the operation by UBM-biometry. Follow-up was 4-9 (average 6.5 ± 1.6) months.

Results:

All operations were non-complicated. Both implants were in the correct position in the CB and changed its preoperative shape and volume. UBM in the meridians of the implants longitudinal axes showed no CB ovalization, as well as a stable position of implants for entire follow-up. Both implants weren’t contacted with each other. PTHIOL in all cases had close contact with the CB without folds and PCO formation. Visual acuity with correction was 0.34±0.13 in three days after surgery, 0.42±0.14 in 1 month and 0.61±0.18 in 3 months. Target refraction was achieved with a deviation of no more than ±0.75 D.

Conclusions:

The study was found: absence of specific complications during and after surgery; method compliance with the requirements of microinvasive lens surgery; its effectiveness to keep CB form and the anatomical and topographical relations in the eye after native lens removal that proves its feasibility in the surgical treatment of cataracts. The method of CBE implantation with simultaneous intraocular correction of aphakia by PTHIOL doesn’t affect the position of the lens and also provides a larger contact area and density of the implants with CB, which may have a positive effect on the PCO frequency and CB fibrosis.
