Phacoemulsification of cataract with implantation of a toric intraocular lens after penetrating keratoplasty

Purpose:
To estimate the possibility of high degree astigmatism correction after penetrating keratoplasty
Setting:
Republican Clinical Eye Hospital, Kazan, Russia
Methods:
A 38 year patient had bilateral penetrating keratoplasty after corneal ulcer with perforation of both eyes 5 years ago. Phacoemulsification with implantation of a toric intraocular lens SN60AT9 (Alcon, USA) was performed on the left eye in 2015. Visual acuity before operation was 20/63 with correction -3,0cyl -6.0ax 75, cells of posterior cornea epithelium were 14890/mm2. IOL power calculation was performed by: WaveLight Oculyzer II keraptotopograph (AG, Germany), IOL Master Biometry (Karl Zeiss, Germany) and Toric Intraocular Lens Calculator (Alcon, USA). Planned refraction - 2,0 Diopters. Cataract Extraction was scleral cut. The course of operation was regular, with no peculiarities.
Results:
Spheroequivalent was +0,25 -2,25 ах88, UCVA 20/40 one month after operation. One year after operation the transplant remained transparent, spherical equivalent was -2,25 -0,7 ах75, BCDVA 20/32. Quantity of posterior epithelium cells was 1400/mm2.
Conclusions:
Implantation of a toric intraocular lens after penetrating keratoplasty permits successful correction of high astigmatism. The advantage of this procedure in comparison with the keratorefractive options is in its minor manipulation on the allograft.
