Abstract Details
Purpose:
To study anterior chamber inflammation, intraocular pressure changes, incidence of cystoid macular edema and endophthalmitis in uncomplicated phacoemulsification surgery cases where intravitreal triamcinolone acetonide and intravitreal plus intracameral ceftazidime injections were given at the end of the surgery and no topical medications were given post operatively. To study the safety of transzonular route of intravitreal injection. To study the acceptance of dropless catract surgery among patients in a private set up.
Setting:
A tertiary care private setup with high volume phacoemulsification surgeries located in Rajkot city, Gujarat, India.
Methods:
A prospective interventional study was carried out in phacoemulsification cases in 400 eyes. Peroperatively intravitreal antibiotic steroid combination was injected via transzonular route after PCIOL implantation and intracameral antibiotic was injected at end of the surgery. 0.12 ml of intravitreal injection comprised of 3.6 mg/ 0.09 ml of preservative free triamcinolone acetonide (Aurocort) and 2.25 mg/0.03 ml of ceftazidime(Cefazid). Intracameral injection comprised of 1 mg/0.1 ml of ceftazidime. The patients were not given topical eye drops post operatively. Rate of anterior chamber inflammation, intraocular pressure, central macular thickness and patient’s comfort level were assessed till 3 months post-operative period.
Results:
Incidence of postoperative endophthlamitis was zero in our study.
No patient had high intraocular pressure till three months post operatively.
12 out of 400 patients had breakthrough anterior chamber inflammation at one month post operative period, of which 6 patients had uncontrolled diabetes.
8 patients had cystoid macular edema of which 2 had uncontrolled diabetes mellitus.
30 patients needed placebo therapy for minor complaints like burning sensation and foreign body sensation.
Conclusions:
Transzonular route of intravitreal injection is a safe method of drug delivery during phacoemulsification surgery.
Post operative dropless cataract surgery is a safe and effective way of endohthalmitis prophylaxis after cataract surgery.
By this simple technique, common complications due to postoperative drops can be easily avoided.
