Changes in choroidal and retinal thickness after cataract surgery

Abstract Details
Purpose:
To evaluate the impact of cataract surgery on subfoveal choroidal thickness and central retinal thickness in the elderly.
Setting:
A prospective study in a clinic for eye diseases, Aleksandrovska Hospital, Sofia, Bulgaria Department of ophthalmology, Medical University, Sofia, Bulgaria
Methods:
20 eyes(20 people) with senile cataract were included in this study, with no previous ocular surgery. Two had age related “dry” macular degeneration and other two were with compensated open-angled glaucoma. All 20 participants undergone uneventful cataract extraction by phacoemulsification and PC-IOL implantation.All surgeries were done by one surgeon and IOL’s were the same for every patient- hydrophobic acrilyc one-piece. A detailed ophthalmologic exam, IOP and SD-OCT measurements were done to every patient prior surgery for baseline and the same were repeated on the 1st day, 7th day and at 1st month.
Results:
According to our preliminary results SFCT showed negative correlation with age and axial length and also increased after the surgery .On the one hand the increase was most significant at 7D and less at 30D and on the other hand there wasn’t a dramatic change at 1D comparing to the baseline.
Conclusions:
Our study has shown that the SFCT increases after cataract surgery despite prophylactic antiinflammatory eye drops, showing that cataract surgery may induces inflammatory changes within the choroid. Phacoemulsification seems to be able to induce morphologic changes in the choroid and retinal layers. However, how cataract surgery induces retinal and choroidal inflammation is not well understood, despite some recent papers which discovered upregulation of numerous inflammation mediators in the retina and choroid . Of course we should consider the lowering effect of used anti-inflammatory eye drops on retinal and choroid thickness.
