Effect of novel non-penetrating opposite clear corneal incision on corneal astigmatism and biomechanical properties during phacoemulsification

Abstract Details
Purpose:
To analyze the outcomes in correction of pre-existing astigmatism and biomechanical properties using Non penetrating OCCI (opposite clear corneal incision) in comparison to conventional Penetrating OCCI.
Setting:
Tertiary eye care centre in Banglore, India
Methods:
Thirty one patients with against the rule astigmatism with more than 1.0 D were randomized into two groups. All patients underwent uneventful clear corneal phacoemulsification on steep axis [which was measured by Pentacam and correlated with IOL master] using 2.8mm keratome. In Group A (16 patient) Conventional OCCI was made using 2.8mm keratome. In Group B (15 patients) biplanar non penetrating OCCI without anterior chamber entry was made. BCVA, pentacam and corvis were recorded for all patients pre-operatively and 1 month post-operatively.
Results:
In group A, mean pre-operative astigmatism was 1.5(+/-0.7SD) which reduced to 0.6(+/-0.25SD) post -operatively. In group B pre-operative mean astigmatism was 1.4 D (+/-0.3SD) which decreased to 0.7(+/-0.2SD) post-operatively. Mean surgically induced astigmatism was 0.25(+/-0.3), measured by vector analysis. Post-operatively in group A , 9 patients had deformation amplitude of more than 1.2 and in group B ,5 patients had more than 1.2 ,p=0.01 i.e it is statistically significant. There was no difference in best corrected visual acuity between the groups.
Conclusions:
There was statistically significant comparable reduction in astigmatism post surgery in group A and group B. However this Novel technique of Non penetrating OCCI has an edge over conventional OCCI due to the benefits of better retention of biomechanical properties of the cornea.
