Improving surgical predictibility in posterior polar cataract

Abstract Details
Purpose:
Surgical management of posterior Polar Cataract is a matter of concern due to its unpredictable course. The purpose of this study is to reduce intraoperative complication by assessing plaque component and posterior capsule integrity by using a high definition anterior segment OCT.
Setting:
Patients were screened and investigated at Holy Family Hospital and Renu eye centre, Patna, Bihar, India.
Methods:
In a prospective study 21 eyes of 16 patients were selected based on clinical diagnosis made on slit lamp examination under full mydriasis. They were subjected to further evaluation by anterior segment HD AS OCT, anterior segment photograph, slit lamp imaging and surgical recording.
Results:
None of the selected cases had any evidence of posterior capsule leakage or defect on Slit lamp.A clear space between discoid plaque and capsule was seen in 20 out of 21 eyes on HD AS OCT..Complete Posterior capsular integrity was present in all 21 eyes. Intra operative course was uneventful in all eyes undergoing slow motion phacoemulsification.
Conclusions:
In posterior Polar Cataract, preoperative assessment of plaque configuration and posterior capsular status by anterior segment OCT imaging can help the surgeon plan accordingly for safe surgical outcome,
