Abstract Details
Purpose:
To describe a technique of nucleus management without cortical cleaving without hydrodissection
Setting:
Dr Om Parkash Eye Institute , Amritsar, India
Methods:
No corticocleaving hydrodissection was performed in 600 eyes with different grades of cataract.[Group 1(200 eyes)Nuclear sclerosis{NS} Grade 1, Group 2(200 eyes)NS Grade 2-3, Group 3 (200 eyes ) mature white/ NS Grade 4] undergoing phacoemulsification from September to December 2015. After capsulorhexis, direct chop was done opposite the main incision.Nuclear chopping was continued 2 clock hours left and right of the initial chop . The two small nuclear pieces were emulsified. The passing of the fluid from the empty bag portion allowed separation of cortex from capsular bag. Nucleus rotation was systematically done with gentle manoeuvre and nuclear emulsification completed.
Results:
Successful nucleus rotation without compromising capsular bag was achieved in189 eyes in Group 1 and all eyes in Group 2 & 3. Of the 600 eyes undergoing phacoemulsification with no hydrodissection technique, none of the eye had posterior capsular rupture
Conclusions:
This technique prevents hydrodissection related complications such as posterior capsule rupture, capsular block and reduces stress on zonules maintaining the capsular bag integrity.
