Dual laser cataract surgery: an experience of the first 100 cases

Purpose:
The purpose of this work is to report our experience using Racot (VI' model of Nd: YAG laser λ=1,44 µm, St.Petersberg, Russia) and femtosecond laser (Victus femtosecond laser platform, Bausch & Lomb) and to provide a sample of the performance and safety results using this new technology.
Setting:
The S. Fyodorov Eye Microsurgery Federal State Institution. St. Petersburg Branch , St. Petersburg, Russia.
Methods:
100 eyes of 94 patients were subjected to cataract extraction. In five cases cataracts were complicated with Fuchs dystrophy and in seven cases patients had open-angle glaucoma. Patients were evaluated preoperatively with the Lens Opacities Classification System III (LOCS III) and get ucorrected and corrected distance visual acuity, biometry, corneal microscopy,anterior segment biomicroscopy, fundus evaluation. Capsulotomy and lens fragmentation were performed with the aid of the Victus femtosecond laser system. The complete lens fragmentation and removal was performed with Nd: YAG laser. All femosecond laser pretreatment and laser cataract extraction procedures were perfomed by the same surgeon.
Results:
Successful femtosecond laser-assisted capsulotomy of 4.5-5.5 mm in a precise position were achieved in all cases. Combination of radial (4-6) and circular (4-6) patterns were used. Femtosecond-laser energy was depended on nuclear density and varied from 7800 to 8500 nJ. “Crater” and “bowel” technique were used during Nd:Yag laser part of the surgery with 130-220 mJ energy for nuclear crush. Posterior capsule rupture during Nd:Yag laser part was observed in one case. 96% of patients had CDVA 20 / 25 or better one day after surgery, others 4% - 20 / 40 or better.
Conclusions:
Femtosecond laser-assisted capsulotomy and lens fragmentation in combination with Nd: YAG laser λ=1,44 µm cataract extraction is new, safe and effective technology.
