Corneal status following dual laser cataract extraction

Abstract Details

Purpose:

To evaluate the amount of endothelial cell loss (ECL) in eyes after laser cataract extraction (LCE) ('Racot VI' model of Nd: YAG laser λ=1,44 µm, St.Petersberg, Russia) with femtosecond laser assisted pretreatment (FLAS) (Victus femtosecond laser platform, Bausch & Lomb).

Setting:

The S. Fyodorov Eye Microsurgery Federal State Institution. St. Petersburg Branch , St. Petersburg, Russia.

Methods:

Femtosecond-laser assisted Nd:YAG laser cataract extraction were performed in 75 patients (81 eyes). Ucorrected and corrected distance visual acuity, biometry, anterior segment biomicroscopy were performed in all cases before and after surgery. Corneal endothelial microscopy (endothelial cell density (ECD), coefficient of variation (CoV) in cell size and percentage of hexagonalitty), corneal pachymetry were perfomed before, 1 day, 1 week, 1 month and 6 month after surgery. The Lens Opacities Classification System (LOCS) III was used for cataract grading. All femosecond laser pretreatment and laser cataract extraction procedures were perfomed by the same surgeon.

Results:

Laser-assisted capsulotomy of 4.5-5.5 mm in a precise position were achieved in all cases. Femtosecond-laser energy was depended on nuclear density and varied from 7800 to 8500 nJ (radial 4 and circular 4-6 patterns). “Crater” and “bowel” technique were used during Nd:Yag laser part of the surgery with 130-220 mJ energy for nuclear crush. The change in corneal pachymetry was not significant. The mean endothelial cell density decreased about 6,31 % on first day and 3,47 % in 6 month. There were no statistically proven difference in the percentage of hexagonal cells before (57.85±5.52) and after (56.23 ±5.53) the surgery.

Conclusions:

Endothelial cell damage after femtosecond-laser assisted Nd:YAG laser cataract extraction is within acceptable limits.
