Comparison of changes in anterior and posterior corneal astigmatism following cataract surgery with and without limbal relaxing incision

Abstract Details

Purpose:

Compare changes in anterior and posterior corneal keratometry in patients undergoing cataract surgery with either limbal relaxing incisions (LRIs) or toric intraocular lens (IOL) implantation.

Setting:

Brighton & Sussex University Hospitals NHS trust

Methods:

This prospective randomised study assessed 70 eyes of 70 patients with pre-operative corneal astigmatism between 0.75 – 2.50 diopters. Patients were randomly allocated to 2 intervention groups; LRI (n=37) and toric IOL (n=33). All patients had a post-operative follow up at 1, 3, 6 and 12 months and mean anterior and posterior keratometry (Kmean) were obtained from Pentacam and compared between both groups. Anterior Kmean between 1 millimetre and 7 millimetre zone from Holladay EKR was also recorded. Data was divided based on two groups of astigmatism: 0.5-1.5D and >1.5D-2.5D.

Results:

Anterior mean keratometry was significantly less in toric IOL group at 12 months only in the group with astigmatism between 0.5-1.5D. On further analysis, this was significantly less for toric group in 6 and 7 mm corneal zones at 12 months (p<0.05). In the group with astigmatism >1.5-2.5D, there was no significant different in anterior Kmean at 1, 3, 6 and 12 months. There was no significant difference in posterior Kmean at any follow up visits for both the astigmatism subgroups.

Conclusions:

In the group with mild astigmatism (0.75-1.50D) there is no difference in anterior Kmean until 12 months where LRIs leads to significant steeper keratometry in peripheral anterior cornea. There does not appear to be any difference in anterior Kmean in those with higher astigmatism (>1.50-2.50D). There is no difference in posterior Kmean at 1, 3, 6 and 12 months for those with astigmatism 0.75-2.50D.
